    FORMFIELDTYPE_xx_yy VARCHAR SELECTION/RADIO The first part is the data type, such as PHONE, EMAIL, SELECTION (see APPENDIX A for list), the second part (after the /) is the form field control used to make the entry,such as TEXT, SELECT, RADIO, CHECKBOX (see APPENDIX B for list)
    FORMFIELDNAME_xx_yy VARCHAR SELECTION_aa_bb The actual 'Name' of the form field that was submitted in the Add to Cart form, containing the selection answer
    ADDITIONAL CUSTOMER SELECTION FIELDS - ffname equals the value of the FORMFIELDNAME_xx_yy Variable. Each ffname is unique in an item, so the xx qualifier is not needed here. Used to reference the actual form field name for the answers, if you wish.
